GAFC Program
Onyx Home Healthcare’s GAFC program was established in 2021 and is designed to serve residents of subsidized housing, with the goal of providing the support services they may need to help them to remain living in their own homes. Onyx Home Healthcare is a Medicaid Group Adult Foster Care provider in Massachusetts, serving elders, and people with disabilities throughout the state!
Activities of Daily Living Covered by GAFC
Participants receive assistance with the following ADLs:
- Bathing
- Personal hygiene / grooming (e.g., hair care, shaving)
- Dressing and undressing
- Eating (includes help with feeding if needed)
- Mobility (e.g., walking, transferring)
- Toileting (including continence-related assistance)
These ADLs are the core functional areas MassHealth evaluates for GAFC eligibility and service delivery.
Instrumental Activities of Daily Living (IADLs)
GAFC also includes assistance with IADLs that support independent living:
- Household-management tasks / general home care
- Laundry
- Shopping (groceries and essentials)
- Light Housekeeping (cleaning, tidying)
- Meal preparation and cleanup .
- Care and maintenance of wheelchairs and adaptive devices.
- Medication Prompting (reminders to take medications)
- Other medically necessary tasks as determined by the GAFC provider (case‑specific IADLs)
