What is GAFC?
The Group Adult Foster Care Program (GAFC) provides daily assistance with personal care service, activities of daily living, and case management services to elderly and, or disabled Mass Health members who meet the clinical criteria for the program and reside in some type of elderly/disabled housing complex.
Our Services
Personal Care Assistance
- Help with activities of daily living (ADLs) such as bathing, grooming, dressing, eating, and toileting
- Supervision or hands-on cueing for activity completion
Homemaker Services
- Light housekeeping, laundry, meal preparation
Medication Support
- Reminders, administration, and management
Nursing Oversight & Care Coordination
- Regular visits by licensed nurses, case manager review of care plans and health monitoring
Transportation Assistance
- Support for travel to medical appointments or essential errands
Who Qualifies?
You might be eligible if you:
- Are 22 years or older and a MassHealth member
- Require at least one ADL assistance—whether supervision, cueing, or hands-on help.
- Reside in MassHealth-approved assisted living or supportive housing.
- Meet Medicaid income and asset criteria (check specifics with MassHealth).
